How long does it take to receive a birth certificate?: If you are coming into the office to request a birth certificate, the proccess takes about 5-10 minutes. You will walk out with the certificate
If you are applying online or through the mail, once we receive all required information, you can expect it to take about one to two weeks through the mail.

1. What if I don't have a Driver's License?: If you do not have and ID and are getting the birth certificate for yourself, we will ask for two other forms of identification to prove your identity. Other forms of identification that we accept are two of the following items:
1. Signed Social Security Card
2. Bank Card with Name Printed and Signed
3. Police Report
4. Fire Report
5. Employee ID with picture and/or signature
6. Tax Return from previous year only
7. Signed Lease or Loan Agreements
8. Signed Court Documents

2. Do I need to bring my social security card to get my birth certificate?: You do not need to bring the social security card unless you are using it for another form of identification.

3. Can I get a step-child's birth certificate?: No, because a step parent is not listed on the birth certificate, we are unable to issue to them.

5. Can I get my birth certificate from the Tippecanoe County Health Department; If I was born in a different county?: No. We only have Tippecanoe County records in our office. You will need to contact the county in which you were born to receive your birth certificate.

6. Can I order a birth certificate over the phone>: No, because we cannot provide identity over the phone, we cannot issue a birth certificate when calling in. Please stop in, request online or mail in an application with proper identification and payment.

7. What do I need to get a septic permit?: Fill out a septic application, have a soil test, and plot a diagram.

8. Who do you recommend as a septic installer?: We do not recommend anyone, but we have a list of installers we have worked with frequently over the years on our website.

9. Who does soil testing in Tippecanoe County?: There is a current list of Professional Soil Scientist/Classifiers on our website.

10. Where can I get a septic diagram on a property?: The health department can email them or print them off. We do have many septic diagrams, however we do not have every septic diagram in Tippecanoe County.

11. What forms of payment do you accept for Birth or Death Certificates?: Cash, Credit/Debit or Money Order

12. What do I do if I have mold in my house?: he first step is to get a physician’s note stating the air quality in your home is causing health issues. Next bring the note to the health department and we will team you up with an environmentalist.

13. What do I do for a rodent infestation?: If you live in city limits please call Lafayette City Engineer at 765-807-1050. If you live outside of the city limits you can contact the health department at 765-423-9221

14. Is there a list of bed bug complaints on local hotels?: We currently do not have this list on the website. This project is in progress.

15. Who does METH Remediation in Tippecanoe County?: There is a current list of Qualified Inspectors we have dealt with frequently on our website.
There is also a state wide list on their website http://www.in.gov/idem/health/2386.htm.

16. What forms of payment do you accept for Food Permits, Pools, Tattoos?: Exact Cash or Money Orders

17. I want to start a new food business, brick and mortar, food truck, etc?: Visit the 629 N 6th St. Location and bring: Your location in mind, prototype menu, and make an appointment to visit the chief of foods. 765-423-9221 Ext. 3

18. Can I sell food out of my home?: NO, because it violates our Indiana Health Foods Codes.

19. How do I file a food/ resturant complaint?: Contact the Health Department

21. I want to set up at the farmers market as a home based vendor?: Currently only west lafayette allows home based vendors, contact market master for more information as they are not regulated by TCHD.
