What if I don't have a Driver's License?
If you do not have and ID and are getting the birth certificate for yourself, we will ask for two other forms of identification to prove your identity. Other forms of identification that we accept are two of the following items:
1. Signed Social Security Card
2. Bank Card with Name Printed and Signed
3. Police Report
4. Fire Report
5. Employee ID with picture and/or signature
6. Tax Return from previous year only
7. Signed Lease or Loan Agreements
8. Signed Court Documents
