     INDIVIDUALS
    SOLE PROPRIETORSHIPS
    GENERAL PARTNERSHIPS

These forms must be printed on Legal size paper

  1. Amendment of Assumed Business Name (PDF)
  2. Assumed Business Name (PDF)
  3. Dissolution of Assumed Business Name (PDF)

All other business types must register with the Secretary of State   www.in.gov/sos                                      This would include Corporations, LLC, LLP, and Non-Profits
