Immunizations are given on Wednesdays from 8:30 AM to 3:45 PM
by appointment only. (CLOSED 12-1 for Lunch)
Please call to make an appointment 765-423-9221, ext. 1.Due to the demand of immunization record requests we charge 25 cents per record.

- 4 Months: DTaP #2, Hib #2, Polio #2, Prevnar #2, Rotavirus #2, Hepatitis B#2
- 6 Months: DTaP #3, Hib #3, Hepatitis B #3, Prevnar #3, Polio #3
- 12 Months: MMR #1, Varicella#1,Prevnar #4, Hep A#1
- 15 Months: DTaP #4, Hib #4, Lead Screening
- 18 Months: Hep A #2
- 5 Years: DTaP #5, Polio #4, MMR #2, Varicella #2
