Permits, Monitoring and Reporting Forms

Below are excerpts from Appendix B "Forms" of the Tippecanoe County Stormwater Technical Standards Manual. These forms and checklists are required in many instances where stormwater activities are taking place.

  • Application for Stormwater Permit - a helpful checklist of supporting documents and data needed prior to applying for a stormwater permit.
  • Notice of Intent - Submission of the Notice of Intent (NOI) letter constitutes official notice that the project owner is applying for coverage under the National Pollutant Discharge Elimination System (NPDES) Construction General Permit for stormwater discharges associated with construction activity. More information on the NOI submittal can be found on the IDEM website. This must be filed with IDEM a minimum of 48 hours before the start of construction. 
  • Construction Site Inspection Maintenance Log - All sediment and erosion control BMPs shall be inspected and maintained to ensure continued performance of their intended function during construction. Inspection and maintenance shall continue until the entire site has been stabilized and a Notice of Termination (NOT) has been issued. The inspection forms must be retained and made available to the Tippecanoe County Drainage Board upon request.
  • Notice of Termination (NOT) - Submission of the NOT letter constitutes notice to the Commissioner that the project site owner is applying for termination of coverage under the National Pollutant Discharge Elimination System (NPDES) Construction General Permit for stormwater discharges associated with construction activity. All construction sites shall undergo a final inspection by the Tippecanoe County Drainage Board following submittal of a NOT by the project owner to ensure the site is stabilized and that post construction BMPs have been properly installed.
